Apply For Assistance

To qualify for assistance, a potential client must present the following:

• Valid Photo ID
• Social Security Card(s) – A card must be presented for every member of the household. If social security cards are not available a birth certificate will do.
• Proof of residence - A current utility bill, a current lease agreement, or any other bill with name and current address.
• Proof of Income – This includes any and all income that comes into the home. Current or recent pay stubs, SSI award letters, food stamp award letters, a current bank statement if income is directly deposited into an account, child support
  orders, etc.
